University","bibliographic_titleLang":"en"}]}]},"item_10002_description_5":{"attribute_name":"抄録","attribute_value_mlt":[{"subitem_description":"【背景と目的】特発性大腿骨頭壊死症（idiopathicosteonecrosisoffemoralhead：以下IONと略す）は，治療方法が未確立で難治性の側面をもち，予後に障害を残す特定疾患＊のひとつである。このような障害をもつ患者の支援方法として，本人から評価されたニーズを重視し，アセスメントや介入を行うことが支援上効果的であるとされている。そこで，本研究では，ION患者のニーズを測定する質問紙調査票（疾患特異性尺度）を開発すると同時に，その尺度の信頼性と妥当性の検討を行なうことを目的とした。\n【研究方法】対象は北海道A・B病院に過去５年間通院履歴のあるION患者６０１名。予備調査として行った個別聞き取り調査およびフォーカスグループインタビュー結果を参考に情報提供に関するニーズ６項目，治療に関する意思決定を行うニーズ８項目，心理面への支援を受けるニーズ１４項目，医療と社会福祉に関するニーズ５項目の合計３６項目を作成し，２０１１年４月郵送法による無記名自記式質問紙調査を実施した。\n【倫理的配慮】本研究は北海道大学医の倫理員会臨床研究審査専門委員会の審査を平成２１年６月１９日付で受けている。\n【結果】有効回答数２９７票（有効回答率９４.３％）であった。作成した３６項目で探索的因子分析（最尤法，プロマックス回転）を行った結果，「良好なコミュニケーション」（１３項目），「精神的安定感」（１４項目），「医療者の全人的対応能力」（９項目）の３因子３６項目からなる尺度が得られた。また，多特性・多方法行列を作成し検討した結果，収束妥当性と弁別妥当性が確認された。３因子それぞれのCronbach'sαは０.９５７～０.９７１と高く，尺度の信頼性が確認された。さらに各質問項目のItem-Total相関分析を行った結果，良好なコミュニケーションで，r=０.６８８～０.９１６，精神的安定感で，r=０.６４４～０.８６３，医療者の全人的対応でr=０.７４２～０.９０６という良好な結果が得られた。\n【考察】ION患者における医療ニーズ測定尺度の信頼性および妥当性は概ね確認され，十分に使用可能であると考えられた。\n＊特定疾患（とくていしっかん）とは、日本において厚生労働省が実施する難治性疾患克服研究事業の臨床調査研究分野の対象に指定された疾患をさしており、難病（なんびょう）とも称される。２０１４年（平成２６年）には、難病の患者に対する医療等に関する法律（難病法）が成立し、医療費の自己負担の軽減を受けられる疾患は、特定疾患から指定難病に移行した。","subitem_description_language":"ja","subitem_description_type":"Abstract"}]},"item_10002_publisher_8":{"attribute_name":"出版者","attribute_value_mlt":[{"subitem_publisher":"旭川大学保健福祉学部","subitem_publisher_language":"ja"}]},"item_10002_source_id_11":{"attribute_name":"書誌レコードID","attribute_value_mlt":[{"subitem_source_identifier":"AA12396607","subitem_source_identifier_type":"NCID"}]},"item_10002_source_id_9":{"attribute_name":"ISSN","attribute_value_mlt":[{"subitem_source_identifier":"18837247","subitem_source_identifier_type":"PISSN"}]},"item_10002_version_type_20":{"attribute_name":"著者版フラグ","attribute_value_mlt":[{"subitem_version_resource":"http://purl.org/coar/version/c_970fb48d4fbd8a85","subitem_version_type":"VoR"}]},"item_creator":{"attribute_name":"著者","attribute_type":"creator","attribute_value_mlt":[{"creatorNames":[{"creatorName":"羽原,
